Day 12 - Wed, Jun 19, 2025

Today I was in triage, nothing really too exciting.  

I asked the doctor to take some skin cancer off my face, and he was too nervous to do it.  So I walked Brendan through what to do, and he did it without a problem 😂

Then the doctor asked me if I wanted to put in some IV's, and between my hand tremors and my lack of being an actual doctor, I passed. I'm confident I could do one, but one of my <many> character flaws is that I pretty much think I can do anything if I've watched a YouTube video.   But I 100% give injections better than they do, though, cause they still sink a 3" needle in the patients' bumm like they used to do when I was a kid.  Most people are ready to pass out.  I do an IM injection in their arm or thigh and they look at me like wait, wtf, that was it?  The biggest problem is when I asked for a 3/4-in #26 needle, they were like "I don't even know if we have those".

The other interesting thing ... Here when they put in an IV, they use about a 2-in needle that they leave in the vein, usually in the back of the hand.  And most people need antibiotics for at least 3 days, some for a week, and they just leave that whole IV rig taped down to their hand.  It's great when they come in, cuz we just have to plug in the IV bag; but then they have to walk around all day with that rig hanging off of them.  I'm surprised it doesn't create all kinds of issues 😳

Finally, they're transferring diabetic foot lady to the hospital tomorrow, so I went and said my goodbyes.  Also to her foot.  She was always remarkable pleasant all things considered, she was always happy to see me, and her whole family was usually up there every day with her.  Not to mention she was my first patient, so there was some sentimental value.  I did one last exam, but i didn't have the heart to tell her what her future held... But let's just say her nickname is gonna be Aileen.  I brought the grandkids some candy and wished her the best 😥

 I left Denver yesterday at 2p, and first stop was Mexico City.  I had an 8 hour layover, which actually went by pretty quick since I had to go through Mexican customs, leave the airport, and then come back in.  Don't ask me why.  And Mexico has a new tax of $50 on foreigners just to step foot in their airport, don't ask me why ... Though I can probably guess.  Then off to Lima for a quick 6 hour flight.  I really, really wanted to sleep on this flight, cause it was about my only chance to sleep, so I took a "sleeping aid" about a half hour before the flight.  Nothing.  So I took another.  Then another.  I stopped at 4 but never did sleep, though I was feeling pretty good at that point. Today there wasn't a ton of excitement, except ... It was just me and one nurse in the ER today, and at some point she said "I'll be back in a while."  So literally for like an hour and a half I was in the ER by myself. Which isn't a huge deal because there's only four beds, but it's kind of a huge deal when I'm mixing meds and putting in IV's when I'm not really familiar with their medications or wtf I'm doing, if I want to get specific.  At one point all four beds were full, and I was running back and forth  and acting all doctor like.  But at least I didn't kill anyone... Yet. The funny thing is my local friend told me last night that everyone in town knows who I am, the gringo doctor.  And when they come to the clinic they try to get me. 😳. I think for the most part it's only because I have some empathy and spend some time with them, and the other doctors seriously just kind of yell at them.  "I told you to take antibiotic...

Every day you finish and go wtf, it couldn't possibly get any crazier.  But then it does.  So today, Brendan, the other guy in the medical program who is a first year student, wanted to do triage so they put me back in the ED. When I got there, it was me and one other doctor, and two nurses.  They told me there was a woman in labor in the operating room/l&d room/lunch room, so I went down to watch (hoping I could tap in). Well shit howdy, I'm not even sure I can put the visual image into words... First, maybe one or two of the nurses had gloves on. I don't think anyone, including the doctor, had a mask on.  There was no monitor on the mom, or the baby, no actual equipment in the room, and for all intents and purposes she might as well been having this kid in a field.  When she had a contraction, a nurse would get on a step stool and literally push down from the top with all her weight, almost like CPR.  The doctor was trying to dilate her with his index...
